4. Household Membership - $30.00 (USD) Subscription period: 1 year Automatic renewal (recurring payments) Join with another individual member to receive membership benefits for both people, but share one set of publications
that will arrive at one address. Please provide the name of the individual member with whom you are joining.

4. Institutional Sponsor Dues - $130.00 (USD) Subscription period: 1 year Automatic renewal (recurring payments) Available to educational institutions; provides three (3) copies of quarterly (4) editions of JPH (mailed to a single address),
three (3) 20% member-discounts on registration fees at biennial national conferences sponsored by the Society, and subscription to the society’s newsletter.

5. Corporate Sponsor Dues - $250.00 (USD) Subscription period: 1 year Automatic renewal (recurring payments) Provides a subscription to the quarterly (4) editions of JPH, three (3) 20% member-discounts on registration fees at biennial national conferences sponsored by the Society, and a subscription to the society’s newsletter.
